What happens when a person that approved something in a chain dies or leaves the country? What happens if someone uses a dead person"s identity (like in voting) to approve links in a chain?

how do you trust the tangerine seller (or one of the many middlemen involved in transacting the produce between grower and final consumer) to not put someone else's QR code on their tangerine…how would a consumer know the QR code is not a copy?
